Yoek Mok, RN serves as the Director of Patient Care Services for Heartford Hospice. In her role, Yok provides overall direction and oversight for the hospice clinical team. She establishes, implements, and evaluates goals and objectives for hospice services that meet and promote the standards of quality and regulatory compliance. Yoek is a registered nurse with over 13 years of experience in hospice serving in roles ranging from RN case manager, Clinical Program Manager, Quality Manager, and National Director of Clinical Services. She has led several hospice agencies successfully through the survey accreditation process. She is a compassionate, hands-on leader who goes above and beyond to support her team, patients, and their families. During her personal time, Yoek loves to spend time gardening and travelling.
